When you hover over the checkboxes there appears a Tooltip. The first box is generally "Linebreak before" and the 2nd box is for "Linebreak after"

... and please remove the linebreak before "ON"

I can't see any AND/OR keywords in the ON clauses. Please add a few AND/OR clauses and it should work
